About the Role: As a Senior Home Health Nurse with Your Tailor Made Senior Service, you will play a critical role in delivering high-quality, patient-centered care to individuals in the comfort of their own homes. This is a dynamic and challenging position that requires a skilled nurse with a strong commitment to providing exceptional care and working collaboratively with interdisciplinary teams.

The ideal candidate will possess a valid RN license in Texas, OASIS/Medicare Certified Visits experience, and electronic charting skills with Finale Health. You will have excellent communication and problem-solving skills, as well as a strong commitment to providing compassionate, individualized care to each patient. Key Responsibilities:
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Conduct thorough nursing assessments to identify patient needs and develop effective care strategies.
- Collaborate with interdisciplinary teams to ensure seamless coordination of care and services.
- Maintain accurate records of all communications with patients, families, physicians, case managers, pharmacies, and other relevant parties.
- Monitor patient services utilization and progress, reporting findings to the care team, patients, family members, physicians, and case managers as needed.
