Job Summary:

We are seeking a skilled Nurse Care Coordinator to join our team. As a key member of our healthcare staff, you will be responsible for providing high-quality patient care and supporting individuals with intellectual, developmental or cognitive disabilities.

ResCare Community Living has decades of experience in the disability services field, providing support to thousands of people across 27 states. Our comprehensive range of services includes community living, adult host homes, behavioral/mental health support, in-home pharmacy solutions, telecare, supported employment and training programs, and day programs.

Essential Job Responsibilities:
* Review daily appointment needs and ensure completion of packets for staff
* Monitor staff and review daily nursing calendar, schedule, and attend if possible
* Implement changes of orders and complete notes of services provided
* Approve orders for prescribed medications/treatments
* Address acute/on-going health issues, including change of condition documentation
* Attend interdisciplinary planning meetings as scheduled
* Provide on-call service and manage resources
* Perform PRN assessments: falls risk, skin integrity, choking
* Develop and update medication support plans

Qualifications:
* Degree from an accredited school of nursing OR current license as a Registered Nurse
* Valid driver's license from state of residence
* One year of nursing experience, supervisory experience preferred

Estimated Salary:
$33.00 per hour USD
