Job Title: Client Care Navigator

Isaiah House is seeking a dedicated and driven individual to fill the role of Client Care Navigator. This position will be based in Shelby County, KY, and serve surrounding counties.

Job Summary:

The Client Care Navigator will be responsible for building strong relationships with Centers of Influence (COI) in their assigned region, educating them on Isaiah House's treatment programs and outcomes, and managing travel schedules to assigned counties.

Key Responsibilities:
  • Establish and maintain relationships with COIs in the assigned region
  • Educate COIs on Isaiah House's treatment programs and outcomes
  • Field calls from potential clients exploring treatment options
  • Manage travel schedules to assigned counties
  • Maintain a database of COI activity, client referrals, and client admissions in ASANA
  • Communicate new client referrals through MS Teams
Requirements:
  • High school diploma or equivalent preferred
  • Knowledge of addiction and substance abuse treatment
  • Connections with community services, organizations, and public groups are essential
  • If in recovery, must demonstrate one year of continuous sobriety
