Job Summary

Centerstone is a leading nonprofit behavioral health system seeking a skilled Care Specialist to join our team. As a Care Specialist, you will provide person-centered assistance to clients in developing life skills and personal goals, while coordinating client care with a multidisciplinary team.

Key Responsibilities
  • Develop and maintain essential elements of the client's clinical record, including treatment plans, progress notes, and reporting.
  • Teach and train clients in activities of daily living, providing services according to organizational guidelines and protocols.
  • Connect clients with community support systems, maintaining regular communications with schools, OFC's, and other family service providers.
  • Provide telephone and on-site crisis intervention/prevention and follow-up to clients and their families.
Requirements
  • Associate's degree required; two years of experience or two years of college coursework in a related field may be substituted for degree requirement.
  • Two years of professional experience working directly with youth and families in a child welfare, school, childcare, criminal justice, behavioral health, or related setting preferred.
