Case Manager

We are seeking a highly skilled and compassionate Case Manager to join our team at Elevance Health. As a key member of our care coordination team, you will play a vital role in ensuring that our members receive the highest quality care and support.
Key Responsibilities- Conduct thorough assessments of members' needs and develop personalized care plans to address their physical, behavioral, and long-term support needs.
- Collaborate with members, caregivers, healthcare providers, and other stakeholders to ensure seamless care coordination and effective communication.
- Identify and address gaps in care, including potential clinical health care needs, and coordinate with the clinical healthcare management and interdisciplinary team to provide care coordination support.
- Manage non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities, to ensure cost-effective and efficient utilization of long-term services and supports.
- Document members' short and long-term service and support goals in collaboration with their chosen care team.
- Identify members who may benefit from alternative levels of service or other waiver programs.
- Participate in process improvement initiatives and serve as a mentor, subject matter expert, or preceptor for new staff.
- Bachelor's degree in a healthcare-related field or a related field with a minimum of 2 years of experience working with a social work agency.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Ability to travel to worksites and other locations as necessary.

We operate in a Hybrid Workforce Strategy, with associates required to work at an Elevance Health location at least once per week. Specific requirements and expectations for time onsite will be discussed as part of the hiring process. Candidates must reside within 50 miles or 1-hour commute each way of a relevant Elevance Health location.
