Adult Case Manager

At Mental Health America of South Central Kansas, we are seeking a dedicated Adult Case Manager to provide targeted case management and community psychiatric support treatment for consumers. As a key member of our team, you will be responsible for assessing and collaborating with consumers to develop personalized goals, ensuring they receive the necessary services and support to achieve their highest level of functioning.
Key Responsibilities:
- Provide case management services to maintain required productivity and billing standards.
- Meet deadlines and ensure accuracy of reports, paperwork, mileage sheets, and timesheets.
- Maintain accurate and timely documentation of service provision, including progress notes that reflect individualized interventions and progress towards goals.
- Submit required progress notes and billing information in a timely manner, adhering to agency, MCO/Medicaid, and COMCARE guidelines and contracts.
- Assist consumers in setting recovery goals using the strengths assessment process and update quarterly.
- Develop and update treatment plans based on identified goals from the strengths assessment.
- Employ recovery strategies in working with consumers, utilizing evidence-based practice tools.
- Assist consumers in achieving educational, employment, and independent living goals.
- Ensure consumers meet their Activities of Daily Living (ADL) needs.
- Meet with consumers to provide interventions necessary to meet identified goals and work towards recovery and reintegration.
- Monitor consumer status and provide level of personal and other supports consistent with their status, notifying the physician and Director/Coordinator/Team Lead of any changes.
- Coordinate other services required to meet identified goals, based on consumer needs and level of functioning.
- Monitor progress of other services to evaluate effectiveness and make necessary revisions, documenting progress on progress notes.
- Assist consumers in accessing and maintaining available entitlements, including SSI, SSDI, food stamps, medical cards, and others.
- Assist consumers with crisis situations, accessing crisis supports, and developing a crisis plan.
- Attend and participate in individual, group, and field supervision as established by EBP's and Program Coordinator/Director/Team Lead.
- Adhere to all Evidence-Based Practice guidelines for Strengths-Based Case Management.
Requirements:
To be successful in this role, you must possess a Bachelor's degree or equivalent experience in the human services field. Computer literacy is required, and preferred areas of knowledge include community resources, housing alternatives, and vocational services. Experience working in the community with individuals who have serious and persistent mental illness is also preferred. A valid Kansas driver's license and access to a personal vehicle are required.
Physical Requirements:
This role requires driving for community mobility, typing/data entry, writing, lifting/carrying up to 30 pounds, bending, stooping, and climbing stairs.
