About the Role

We are seeking a compassionate and dedicated Case Manager to join our team at TFI Family Services, Inc. As an Aftercare Case Manager, you will play a vital role in supporting children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aged out youth.

Key Responsibilities
  • Provide support and services to children and families to ensure their stability and well-being
  • Develop and implement aftercare plans to address the unique needs of each family
  • Collaborate with public and private agencies to provide supportive services and resources
  • Conduct regular in-person meetings with children and families to monitor progress and address concerns
  • Document and report on case activities and outcomes in a timely and accurate manner
