As our census continues to grow, we are seeking a compassionate and empathetic Full-Time RN Case Manager to join our Garland Service Area team. Your primary focus will be providing support to individuals and families facing the challenges associated with terminal illnesses and end-of-life care.

Responsibilities:
  • Conduct field visits with the nursing team, coordinating care based on patient needs and family support requirements.
  • Educate patients, families/caregivers, and staff on appropriate medication usage, durable medical equipment (DME), supplies, disease progression, and pain management techniques.
  • Ensure patient comfort by addressing concerns promptly and delivering exceptional care and attention.
  • Demonstrate a deep understanding of the hospice philosophy, pain management principles, and the unique needs of terminally ill patients. Effectively communicate this knowledge in a clear and compassionate manner to patients and families.
  • Exhibit excellent interpersonal and communication skills.

Qualifications:
  • Degree in Nursing (ADN or BSN).
  • Valid RN Texas License.
  • At least 1 year of Hospice experience is preferred.
  • Valid Texas Driver's License and Proof of Auto Insurance.
  • Reliable Transportation.
  • Valid BLS Certification.
  • Successful completion of a Background Screen including employment verification.
