Compensation: OTE $150K - $200K

We are in search of a dedicated and empathetic Primary Care Nurse Practitioner to provide exceptional healthcare services. This position focuses on delivering thorough medical care to patients residing in long-term care settings.

Key Responsibilities:
  • Deliver holistic healthcare services and oversee patient management.
  • Collaborate with a multidisciplinary team of healthcare experts.
  • Evaluate, diagnose, and address both physical and psychological health of residents.
  • Conduct comprehensive patient evaluations, including medical histories and physical examinations.
  • Order and analyze diagnostic tests.
  • Formulate diagnoses and treatment strategies.
  • Prescribe medications and administer necessary treatments.
  • Provide guidance and support to residents and their families.
  • Work closely with the interdisciplinary team to ensure coordinated care.
  • Manage chronic health conditions and track patient progress.
  • Adjust care plans as required to meet patient needs.
  • Commit to a person-centered approach in all care activities.
  • Create a safe and supportive environment for elderly residents.

Qualifications:
  • Active Nurse Practitioner License.
  • DEA License.
  • Minimum of 6 months experience in long-term care settings.
