Job Summary:

The Springs of Red Oak is seeking a highly skilled and compassionate MDS Coordinator RN/LPN to join our team. As a key member of our care team, you will be responsible for promoting the physical and emotional well-being of our residents.

Key Responsibilities:

  • Gather information using the Resident Assessment Instrument (RAI) during resident interviews to assess their functional capabilities and develop individualized care plans.
  • Utilize the Minimum Data Set (MDS) to assess residents' functional capabilities and develop care plans that ensure optimal care.
  • Develop and implement individualized care plans that meet the unique needs of each resident.
  • Ensure compliance with medical and ethical standards, as well as regulatory requirements.
  • Manage MCR/Managed Care MDS updates and care plans to ensure seamless care transitions.
