Join our dedicated Dierksen Hospice team as a PRN Registered Nurse Case Manager (RNCM) and make a significant impact in the lives of patients and their families.

We seek a compassionate, detail-oriented Registered Nurse (RN) to serve as a Hospice Case Manager. In this role, you will plan, organize, and oversee hospice care by utilizing the nursing process, which includes assessment, planning, interventions, implementation, and evaluation. You will effectively engage with patients, caregivers, families, and interdisciplinary team members while upholding professional nursing standards and clinical competency. Key Responsibilities:
  • Empower Through Engagement: Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate approach that reflects our commitment to exceptional service.
  • Collaborative Care Champion: Work closely with our Clinical Director to evaluate the eligibility and suitability of clients for our specialized hospice services, ensuring the right care is provided at the right time.
  • Nurturing Direct Care: As an RN Case Manager, you will not only coordinate care but also serve as a source of comfort and support for patients, prioritizing their comfort and quality of life.
  • Nursing with Heart: Utilize your nursing skills to deliver high-quality care based on proven principles and techniques, helping to alleviate pain and provide solace to those in our care.
  • Educator and Advocate: Empower patients and families through education, encouraging their active involvement in creating personalized care plans that align with their goals.
  • Call of Compassion: Share the responsibility of call duties with our team, demonstrating our commitment to being present when it matters most.
Qualifications:
  • Current unencumbered license as a Registered Nurse.
  • Minimum of one (1) year of experience as an RN in a medical surgical/acute care setting; hospice experience is a plus.
  • Proficient in computer skills, including typing and various programs, with a background in EMR.
  • Current CPR certification is required.
