Transition Coordinator

Connecticut Community Care is seeking a skilled Transition Coordinator to support the transition of eligible individuals in the North Central region. The ideal candidate will have a strong background in human services and excellent communication skills.
Key Responsibilities- Establish and maintain relationships with consumers, families, and community providers to facilitate the transition process.
- Conduct initial interviews and complete intake guides with individuals in skilled nursing facilities.
- Assist consumers in developing and implementing transition plans.
- Follow up with consumers for 365 days after transition.
- Complete and submit applications for Medicaid, food stamps, and/or cash assistance.
- Advocate on behalf of consumers as necessary.
- Coordinate plans and make referrals to community resources.
- Maintain accurate consumer records.
- High School Diploma or equivalent; Bachelor's Degree in Human Services or related field preferred.
- At least one year of experience in a human services setting for Bachelor's Degree holders; at least two years of experience for High School Diploma/equivalent holders.
- Thorough knowledge of community resources.
- Ability to work as part of a team and communicate effectively with consumers, families, and providers.
The Transition Coordinator will work in various environments, including office, client homes, hospitals, nursing homes, and community settings. Reliable transportation and a valid driver's license are required.
