Care Manager

The Care Manager position at Connecticut Community Care is a vital role in assisting individuals in maintaining an interactive process of informed decision-making about Long-Term Services and Supports.
Key Responsibilities- Conduct comprehensive, systematic assessments that are person-centered with individuals, including family or representative as requested, in the person's preferred setting.
- Educate individuals on the components of the program, service options, and DSS guidelines, including eligibility, costs, and how each may work with the person's formal and informal supports and resources.
- Promptly complete all client documentation, applications, forms, and additional documentation as required.
- Monitor and review continued cost effectiveness, quality, and appropriateness of care plan/service delivery, service order entry and renewals, and the contractual obligations.
- Conduct person-centered telephone and in-person interviews with clients and their families, and other activities necessary for reassessment of clients and the monitoring and adjustment of care plans.
- Work effectively as part of an interdisciplinary team and in conjunction with other internal and external resources and committees.
- Bachelor's degree in administration, social work, public health, recreation, psychology, counseling, gerontology, or related field required.
- Must have a minimum of two years' experience in healthcare or human services. A Bachelor's degree in Nursing, Health, Social Work, Gerontology, or a related field may be substituted for one year of experience.
- Knowledge and understanding of psychological, human development, social, health, and economic factors influencing the attitudes and behavior of individuals and families.
- Knowledge of community resources available to individuals and families; an ability to mobilize resources into a coordinated and comprehensive plan of care.
- Familiarity with funding and financial sources; including Medicaid and Medicare.
