Schedule and Work-Life Balance

Job Responsibilities

As an End of Life Care Case Manager, you will:

  • Assume Primary Responsibility: Oversee patient and family caseloads, ensuring seamless coordination of care.
  • Initiate Communication: Collaborate with interdisciplinary teams, attending physicians, and other agencies to deliver optimal care.
  • Provide Regular Visits: Assess patient needs and update care plans to ensure timely interventions.
  • Perform Clinical Pronouncement: Make accurate pronouncements of death and provide emotional support to families.
  • Maintain Accurate Records: Ensure timely documentation of patient records, charts, progress notes, reports, and care plans.

Qualifications

  • Graduate of Accredited Nursing Program: Possess a degree from a recognized nursing program.
  • Licensed Registered Nurse: Hold a valid RN license in the state of practice.
  • CPR and First Aid Certification: Maintain current certifications in CPR and first aid.
  • Valid Driver's License: Possess a valid driver's license and proof of insurance.

Salary Estimate

The estimated annual salary for this role is $80,000-$110,000, based on industry standards and location.
