Job Overview

A Path of Care Home Health is a leading provider of home health care services in Oklahoma. We are currently seeking a skilled Field Nurse (LPN) to join our team.


Compensation and Benefits

The successful candidate will receive a competitive salary, estimated at $55,000 - $65,000 per year, based on experience and location. In addition to the base salary, we offer a generous loyalty bonus of $3,000, as well as great benefits, including medical, dental, vision, short- and long-term disability, paid holidays, paid time off (PTO) accrual, a 401(k), schedule flexibility, and life insurance.


Job Description

As a Field Nurse (LPN) with A Path of Care Home Health, you will play a vital role in delivering high-quality care to our patients in their homes. Your primary responsibilities will include visiting patients to provide hands-on nursing skills, such as injections and wound care, while ensuring equipment and materials are prepared correctly. You will also teach patients appropriate self-care techniques, communicate effectively with RNs and case managers, and maintain accurate documentation of patient information.


Required Skills and Qualifications
  • Active nursing license in the state of Oklahoma
  • CPR certification or the ability to obtain one within 60 days of being hired
  • 1+ years of nursing experience
  • Valid driver's license and reliable transportation
  • Ability to travel long distances and adapt to schedule changes
  • Basic knowledge about computers

Work Schedule

This full-time position typically works from 8 AM - 5 PM, Monday to Friday with rotating weekends. While we understand the importance of work-life balance, we do require our Field Nurses to be flexible and able to adapt to changing schedules as needed.
