Job Summary

VITAS Healthcare is seeking a compassionate and skilled Registered Nurse to join our interdisciplinary team. As a Hospice Care Coordinator, you will play a vital role in providing high-quality, patient-centered care to individuals and their families facing terminal illness and bereavement.

Key Responsibilities
  • Coordinate care and ensure continuous assessment of patients' and families' needs
  • Implement the interdisciplinary plan of care and provide education and training to patients, families, and caregivers
  • Participate in patient care meetings and bereavement support
  • Provide effective communication and shift reports through written, verbal, and body language
  • Work with the Interdisciplinary Group (IDG) to develop, coordinate, and follow the patient's individualized plan of care
  • Obtain physician orders when necessary and provide those services requiring substantial and specialized nursing skill
  • Evaluate the outcome of services and delegate nursing tasks to Hospice Aides, Volunteers, and Licensed Practical/Vocational Nurses
Requirements
  • Currently licensed to practice nursing in the state where the VITAS program is located
  • A minimum of two years of nursing experience in hospice, home health, or community health in the last five years
  • Equivalent experience or licensure may be considered
