About the Position

This RN Care Manager role is designed for a skilled registered nurse who can effectively manage chronic care patients, communicate with healthcare providers, and ensure timely and efficient care delivery. As a Chronic Care Management RN, you will be responsible for developing and implementing care plans, monitoring patient progress, and collaborating with healthcare teams.

Key Responsibilities
  • Develop and implement comprehensive care plans for chronic care patients.
  • Collaborate with healthcare providers to ensure coordinated care.
  • Monitor patient progress and adjust care plans as needed.
  • Communicate with patients and families to provide education and support.

Requirements

To succeed in this role, you should have:

  • A current Alabama Licensed Registered Nurse (RN)
  • 3 years of related care management, medical clinical experience, and/or training
  • Strong analytical and problem-solving skills
