Job Summary:

We are looking for an experienced Nurse Practitioner who will provide comprehensive patient care, including diagnosis, treatment, and health education.

Main Responsibilities:

  • Patient Assessment and Care Planning: Conduct thorough physical exams and assessments to identify patients' health needs and develop personalized care plans.
  • Disease Management: Diagnose and manage acute and chronic conditions, prescribing medications and developing effective treatment plans.
  • Patient Education and Support: Educate patients on healthy habits, disease prevention, and management, promoting patient-centered care.
  • Collaboration and Communication: Work closely with healthcare teams to ensure seamless care coordination and effective communication.

Requirements:

  • A current Nurse Practitioner certification and state license.
  • Experience in a clinical setting is preferred.
