Position Summary

The Lead Case Manager is tasked with overseeing case management for newly arrived refugees during their critical resettlement phase. This role includes making referrals to external services, maintaining accurate documentation, and providing ongoing training to clients. The individual will assist the Program Director in guiding the case management team and ensuring high standards of service delivery.

Key Responsibilities

  • Lead and support case management efforts, providing guidance to case managers.
  • Conduct training sessions as necessary to enhance team capabilities.
  • Manage daily program operations in the absence of the Program Director.
  • Facilitate weekly meetings focused on high-needs clients.
  • Participate in file reviews to promote Continuous Quality Improvement alongside the Program Coordinator.
  • Keep track of and update client financial records accurately.
  • Establish collaborations with public and private entities to foster employment opportunities.
  • Design and implement self-sufficiency and employability strategies for clients.
  • Recruit participants for the Vulnerable Care Services program.
  • Organize and maintain both physical and digital client files efficiently.
  • Provide weekly progress reports during departmental meetings.
  • Offer emotional support and educational resources to clients in the Vulnerable Care Services program.
  • Deliver immediate case management assistance to clients, helping them formulate both short-term and long-term housing and employment plans.
  • Conduct intake assessments for various programs.
  • Engage with clients regularly to monitor their progress and ensure access to essential services.
  • Assist clients in navigating the social services landscape to eliminate barriers to service access.
  • Document all actions and interactions in relevant databases and software.
  • May require driving agency or personal vehicles for work-related tasks.

Qualifications

  • Bachelor's degree in Human Services or a related discipline.
  • A minimum of three years of experience in case management.
  • Certification in CPR and First Aid is required.
  • Proficient in Microsoft Office Suite.
  • Excellent communication skills, both verbal and written, with a focus on clarity and organization.
  • Demonstrated ability to work effectively with diverse families and communities.
  • Physical capability to lift up to 35 lbs. occasionally and perform tasks requiring bending, climbing, and reaching.

Regulatory Requirements

  • Must be at least 18 years old.
  • Possession of a valid driver’s license and proof of insurance.
  • Ability to obtain and maintain Arizona Level One Fingerprint Clearance Card and FBI National Criminal Records History Report.
  • Pass a pre-employment drug screening.

Preferred Qualifications

  • Master's degree in human services or a related field.
  • Additional years of relevant experience.
  • Multilingual capabilities are a plus.
