Role of the RN Case Manager

The RN Case Manager plays a crucial role in facilitating patient care through effective resource coordination, ensuring optimal health outcomes, and promoting appropriate access to care. This position is responsible for:

  • Conducting utilization management to support medical necessity and prevent denials.
  • Managing transitions of care to enhance patient satisfaction and prevent readmissions.
  • Coordinating care to ensure efficiency and appropriateness at every level.
  • Complying with state and federal regulations, accreditation standards, and organizational policies.
  • Providing education to physicians, patients, families, and caregivers.
Key Responsibilities

The RN Case Manager's responsibilities include:

  • Performing accurate medical necessity screenings and submitting for Physician Advisor review.
  • Coordinating care and planning transitions for patients.
  • Leading multidisciplinary patient care conferences.
  • Managing concurrent disputes and making appropriate referrals.
  • Communicating effectively with patients and families regarding care plans.
  • Ensuring timely and thorough documentation in the Case Management system.
Qualifications

The ideal candidate will possess the following:

Education
  • Required: Graduate of an accredited nursing program.
  • Preferred: Academic degree in nursing (bachelor's or master's).
Licensure/Certification
  • Required: Active RN license in the state of practice.
  • Preferred: Accredited Case Manager (ACM) certification.
Experience
  • Required: Minimum of 3 years of relevant department experience or 4 years in acute hospital case management.
