We are currently seeking a skilled and dedicated professional to fill the role of Case Manager 3 within our Early Intervention Unit.
The Division of Developmental Disabilities (DDD) is committed to providing high-quality, flexible services to individuals with developmental disabilities and their families. As a Case Manager, you will play a vital role in facilitating access to essential resources and support systems that promote independence and community engagement.
Key Responsibilities:
- Conduct home visits to establish rapport with families, understanding their unique dynamics, priorities, and concerns while making appropriate referrals to community resources.
- Organize and lead multi-disciplinary Individualized Family Service Plan (IFSP) meetings at least biannually to assist children with developmental disabilities and their families in achieving meaningful outcomes.
- Provide families with essential information regarding their rights and responsibilities, while also offering support during challenging times, including grief and loss.
- Collaborate with the Department of Child Safety (DCS) to ensure the needs of children in the DCS system are met effectively.
- Regularly assess and document each child's progress, adjusting plans as necessary to support their development.
Required Knowledge, Skills, and Abilities:
- Understanding of methods and techniques for developing Individualized Family Service Plans.
- Familiarity with HIPAA regulations and relevant federal and state laws regarding care for individuals with developmental disabilities.
- Proficient in case management principles and practices.
- Strong communication skills, both oral and written, with an emphasis on conflict resolution.
- Ability to work autonomously and collaboratively within diverse teams.
- Capability to lead and support a team of coordinators in the absence of direct supervision.
- Effective organizational skills to manage a caseload efficiently.
- Valid driver's license and the ability to operate a vehicle for work-related duties.
Preferred Qualifications:
- Experience working with individuals with developmental disabilities is highly desirable.
Pre-Employment Requirements:
- A Bachelor's degree in Early Childhood, Special Education, Family Studies, or a closely related field is required.
- Possession of a valid Level One fingerprint clearance card is necessary for working with children and vulnerable adults.
- Successful completion of background checks and reference verification is mandatory.
- All new hires must complete the Electronic Employment Eligibility Verification Program (E-Verify).
