Position SummaryThe Lead Case Manager is responsible for overseeing case management and facilitating referrals for newly arrived refugees during their initial resettlement phase. This includes connecting clients with external services and agencies, documenting provided services, and offering ongoing training to refugees. The individual will assist the Program Director in leading the case management team and support staff. This role may involve high-pressure situations and demands a strong performance level. Additional responsibilities may be assigned as needed.
Key Responsibilities- Provide leadership and support to Case Managers within the program.
- Conduct ongoing training sessions as required.
- Supervise daily program operations in the absence of the Director.
- Facilitate weekly meetings for high-needs clients.
- Conduct file reviews as part of the Continuous Quality Improvement process with the Program Coordinator.
- Maintain and update client financial records.
- Establish partnerships with public and private organizations to enhance employment opportunities.
- Develop, implement, and monitor self-sufficiency and employability plans for clients.
- Recruit participants for the Vulnerable Care Services (VCS) program.
- Organize and maintain both physical and digital client files efficiently.
- Provide weekly reports during departmental meetings.
- Offer emotional support and educational resources to VCS clients.
- Deliver immediate case management services to clients, ensuring they create both short-term and long-term housing and employment plans.
- Complete necessary intake processes for various programs.
- Engage with clients to support their achievement of desired outcomes, conducting monthly check-ins to ensure access to essential needs.
- Assist clients in navigating the social services system to access requested services and eliminate barriers.
- Document all actions taken in relevant databases and software.
- May require driving agency or personal vehicles for business purposes.
Relevant work experience may be considered as transferable skills to meet the minimum qualifications for this position, as determined by the Executive Director of Human Resources.
- Bachelor's degree in Human Services or a related field.
- A minimum of 3 years of experience in case management.
- Certification in CPR and First Aid.
- Proficient in Microsoft Office Suite.
- Strong communication skills, both oral and written, with clarity and organization.
- Demonstrated ability to work effectively with diverse families from various backgrounds.
- Physical ability to lift 35 lbs. occasionally; capable of stooping, bending, climbing, and reaching.
- Must be at least 18 years of age.
- Valid driver's license and proof of insurance.
- Ability to obtain and maintain Arizona Level One Fingerprint Clearance Card and FBI National Criminal Records History Report (employer paid).
- Pass a pre-employment drug screening (employer paid).
- Master's degree in Human Services or a related field.
- Additional years of relevant experience.
- Multilingual capabilities.
