The RN Case Manager will be responsible for:

  • Facilitating patient care through effective resource coordination to ensure optimal health outcomes.
  • Assessing patient needs for transitions to promote timely discharges and prevent unnecessary readmissions.
  • Implementing national standards for case management, including:
    • Utilization Management: Ensuring medical necessity and preventing denials.
    • Transition Management: Supporting appropriate lengths of stay and enhancing patient satisfaction.
    • Care Coordination: Ensuring efficient throughput and appropriate levels of care.
    • Compliance: Adhering to state and federal regulations, accreditation standards, and organizational policies.
Key Responsibilities Include:
  • Conducting accurate medical necessity screenings.
  • Coordinating care and planning transitions.
  • Leading multidisciplinary patient care discussions.
  • Managing disputes and referrals to other departments.
  • Communicating effectively with patients and families regarding care plans.
  • Maintaining thorough documentation in the Case Management system.
Qualifications

The ideal candidate will possess:

  • Education: Graduate of an accredited nursing program; a bachelor's or master's degree in nursing is preferred.
  • Licensure: Active RN license as required by state regulations; accreditation as a Case Manager is preferred.
  • Experience: A minimum of 3 years in department experience or 4 years in acute hospital case management.
