- Provide direct case management services, ensuring effective coordination and monitoring of individual needs.
- Educate clients on various benefits, including Medicaid, Medicare, Social Security, Food Assistance, and Housing Support.
- Conduct thorough assessments to identify the needs of individuals and maintain accurate records.
- Collaborate with clients, families, and support systems to develop personalized goals and objectives.
- Implement and adhere to the Person-Centered Planning process consistently.
- Observe and evaluate the implementation of services and suggest areas for improvement.
- Establish community connections and collaborate with local schools, healthcare providers, and other agencies.
- Ensure compliance with organizational policies and maintain confidentiality at all times.
Supervision: This role does not involve supervisory responsibilities and reports directly to the program supervisor.
Qualifications:
- Education: A Bachelor's Degree in Social Work, Psychology, Human Services, or a related field is required.
- Licensure: A valid driver's license is necessary.
- Experience: Minimum of one year in a relevant field, with two years of experience working with individuals with intellectual and developmental disabilities preferred.
- Skills: Proficiency in Microsoft Office and familiarity with Social Security processes is essential.
- Transportation: Access to reliable transportation is required.
