Key Responsibilities:
  • Provide direct case management services, ensuring effective coordination and monitoring of individual needs.
  • Educate clients on various benefits, including Medicaid, Medicare, Social Security, Food Assistance, and Housing Support.
  • Conduct thorough assessments to identify the needs of individuals and maintain accurate records.
  • Collaborate with clients, families, and support systems to develop personalized goals and objectives.
  • Implement and adhere to the Person-Centered Planning process consistently.
  • Observe and evaluate the implementation of services and suggest areas for improvement.
  • Establish community connections and collaborate with local schools, healthcare providers, and other agencies.
  • Ensure compliance with organizational policies and maintain confidentiality at all times.

Supervision: This role does not involve supervisory responsibilities and reports directly to the program supervisor.

Qualifications:
  • Education: A Bachelor's Degree in Social Work, Psychology, Human Services, or a related field is required.
  • Licensure: A valid driver's license is necessary.
  • Experience: Minimum of one year in a relevant field, with two years of experience working with individuals with intellectual and developmental disabilities preferred.
  • Skills: Proficiency in Microsoft Office and familiarity with Social Security processes is essential.
  • Transportation: Access to reliable transportation is required.
