About the Role

This position requires a Bachelor's degree or equivalent experience, as well as Child Abuse Clearance, CPR Certification, and Criminal Background Clearance. As a Patient Care Coordinator - Mental Health, you will work under nursing supervision, responding to facility Code events and providing crisis intervention when necessary. Your ability to prioritize tasks, gather patient care data, and report changes in patient status will be essential to the success of our team.

Key Responsibilities

- Collaborate with the multidisciplinary team to assess patient needs and develop effective treatment plans (60%)- Organize patient care tasks according to priority and importance (20%)- Obtain vital signs, height, and weight measurements, and respond to facility Code events (10%)- Report changes in physical, mental, and emotional conditions to nursing staff (10%)

Salary Information

The estimated annual salary for this position is between $55,000 and $75,000, depending on qualifications and experience. We also offer a comprehensive benefits package, including medical, dental, and vision insurance, as well as 401(k) matching and paid time off.
