Patient Care Coordinator

This Patient Care Coordinator - Behavioral Health position is an exciting opportunity to join our team at Highmark Health. The salary for this role is approximately $50,000 - $60,000 per year.
The ideal candidate will maintain safety and structure by observing and recording patient information, working under nursing supervision, and collaborating with the multi-disciplinary team to provide ongoing assessments of patient status and gather pertinent care data through rounding and patient interaction.
Responsibilities also include providing crisis intervention on the unit to maintain safety, responding to facility Code events, and reporting any changes in physical, mental, and emotional conditions to nursing staff and suggesting modifications to the master treatment plan.
Applicants should have a Bachelor's degree or one to two years of relevant experience and/or education as determined by the company, as well as 1-3 years of mental health or clinical experience.
