JOB OVERVIEW AND QUALIFICATIONS

Clackamas County's Office of Public Health Emergency Services is on the lookout for a dedicated Community Health Paramedic to enhance our team. This office is charged with overseeing Emergency Medical Services (EMS), Public Health Emergency Preparedness (PHEP), the Medical Reserve Corps (MRC), and public health emergency responses. The Community Health Paramedic plays a crucial role in linking patients to primary healthcare and additional services, ensuring thorough post-hospital follow-up care, and advocating for health education. The most qualified candidates will have hands-on experience in delivering community health services and medical outreach, which includes evaluation and treatment, vaccination initiatives, behavioral health support, management of substance abuse issues, and responses to public health emergencies.

Essential Qualifications and Transferable Skills:

  • A minimum of three (3) years of relevant experience as a practicing Paramedic, equipping you with the necessary knowledge and skills for this role.
  • Valid Oregon Paramedic License (mandatory at the time of hiring).

Preferred Qualifications and Transferable Skills:

  • Certification as a Community Paramedic (CP-C)
  • Certification as a Critical Care Paramedic (CCP-C)
  • Oregon Qualified Mental Health Associate (QMHA) Certification
  • Oregon Community Healthcare Worker Certification
  • Experience in Case Management collaborating with allied health, county health, and social services
  • Bilingual proficiency in English and Spanish

Pre-Employment Requirements:

  • Successful completion of a post-offer, pre-employment drug screening.
  • Passing a criminal history background check, which may include national or state fingerprint checks.
  • Completion of a post-offer physical assessment; accommodation requests will be considered on an individual basis in accordance with State and Federal regulations.
  • Regular driving is required for County business; candidates must possess a valid driver's license and maintain an acceptable driving record throughout employment.

TYPICAL RESPONSIBILITIES

Key Duties Include:

  • Collaborating with community partners on initiatives addressing opioid use, behavioral health issues, frequent 911 callers, and enhancing community access to care; working with other agencies to ensure continuity of patient care.
  • Performing paramedic duties, providing patient care in line with established protocols, medical guidance, and Public Health policies; swiftly assessing medical situations and responding effectively.
  • Conducting health assessments, treatments, and coordinating services for patients; performing initial home evaluations and ongoing visits as needed; facilitating virtual consultations with healthcare providers and arranging transportation as necessary.
  • Executing post-hospital discharge follow-up care, including physical assessments, medication reviews, and monitoring vital signs.
  • Documenting and reporting patient conditions, reactions to treatments, and significant occurrences to the supervising physician.
  • Providing patient education on various health topics, including chronic disease management, nutrition, and fall prevention.
  • Conducting follow-up assessments post-injury, including safety evaluations for home environments.
  • Coordinating appointments and follow-ups with healthcare providers and facilities.
  • Connecting individuals with community resources for additional support.
  • Completing necessary documentation and reports; utilizing records and statistical data for planning and evaluation of responsibilities.
  • In emergency situations, supporting the Public Health Department's Operations Center and other emergency responses, which may involve assisting with vaccination clinics or other field efforts.

REQUIRED KNOWLEDGE AND SKILLS

Comprehensive Knowledge of: Principles, methods, and procedures of paramedic practice; preventative healthcare; community health aspects of paramedic programs; understanding of communicable diseases, chronic illnesses, mental health issues, and other disabling conditions; principles of mental health.

Working Knowledge of: Community resources for public health and human services; current trends in paramedic practice; office equipment and software applications.

Skills Required: Ability to establish and maintain effective relationships with community partners, colleagues, and the public; proficient communication skills, both verbal and written, across diverse situations; analytical skills to assess situations accurately and take appropriate action; recognition of public health issues affecting physical and mental well-being, and assisting in securing solutions; delivering high-quality paramedic care; educating on disease prevention and health promotion; proficiency in operating office equipment and software.

WORKING CONDITIONS

  • Work is performed outdoors in various conditions, including extreme temperatures and inclement weather, with exposure to hazardous materials. Interactions occur with diverse socioeconomic groups, including individuals with mental health challenges and substance abuse issues.
  • Physical demands include exertion and agility, such as bending, kneeling, lifting, and moving objects and individuals.

WORK SCHEDULE

This position participates in the County's alternate workweek program, working 40 hours across a standard workweek from Monday to Thursday (Fridays off). Specific working hours will be discussed with the selected candidate upon extending an employment offer.

This role is eligible for a hybrid work model, combining on-site and telework, based on departmental needs. The exact arrangement will be discussed with the hiring manager during the offer process. Telework locations must be within Oregon or Washington, with Washington residents required to work 50% of their time on-site in Clackamas County.
