Our facility specializes in residential care tailored for individuals with complex medical needs and behavioral challenges. We are in search of a Full-Time LPN Wellness Nurse to collaborate closely with our Director of Nursing in delivering outstanding care to our residents. We welcome clinicians who wish to merge their passion for healthcare with opportunities for professional growth.
Work Schedule: Friday to Monday, 10-hour shifts. Weekend availability is required.

**Eligibility for benefits is contingent upon meeting specific criteria (e.g., hours worked).
The primary duties of the LPN Wellness Nurse encompass, but are not limited to:
- Collaborating with the Director of Admissions to perform pre-admission evaluations and screenings.
- Overseeing and updating residents' Service Plans, particularly during quarterly reviews.
- Conducting evaluations and monitoring significant changes in residents' conditions.
- Assessing and tracking residents' physical and cognitive health.
- Maintaining regular communication with families and external care providers.
- Coordinating with pharmacies, physicians, residents, and families to ensure precise medication management.
- Training, supervising, and motivating the care team to deliver the highest quality of service.
- Administering medications and documenting treatments accurately.
- Performing non-delegated tasks, including Intramuscular Injections.
- Completing additional responsibilities as assigned.
