Job Schedule: Full-Time

We are in search of a NextSteps Case Manager to oversee the coordination and delivery of services for individuals in the Next Steps Sober Independent Living program. As a NextSteps Case Manager, your role will be pivotal in fostering hope and providing tailored case management services.

Key Responsibilities:
  • Exhibit respect and cultural sensitivity while engaging with participants, colleagues, and community partners.
  • Adhere to all policies, procedures, and agreements relevant to the residential and Next Steps programs.
  • Create individualized case plans that align with participant needs and program objectives, establishing actionable steps and tracking progress through regular follow-ups.
  • Conduct thorough assessments of participant requirements and recommend appropriate intervention levels.
  • Complete all necessary documentation for admissions, referrals, assessments, and discharges for residents in the Next Steps program.
  • Regularly evaluate and adjust resident case/action plans as needed, collaborating with the Next Steps team on Life Skills programming.
  • Guide and hold residents accountable to program standards as outlined in the Resident Handbook.
  • Facilitate case/action plan meetings with residents, accommodating both scheduled and walk-in appointments.
  • Oversee case planning sessions, coordinate potential resident interviews, and assess ongoing resident needs to optimize appointment availability.
  • Work collaboratively to identify and address escalating situations, employing de-escalation techniques and trauma-informed care practices.
Qualifications:
  • Bachelor's Degree in human services or a related field is required.
  • A minimum of 2 years of relevant experience is necessary.
  • Understanding of the legal aspects associated with medical interventions is essential.
  • Ability to operate assigned vehicles or personal vehicles, adhering to all applicable laws; proof of liability insurance is required.
  • Physical capability to lift and move objects or individuals as needed.
  • Ability to remain mobile for extended periods, including walking, standing, and sitting.
