About the Role:
- Develop and implement personalized care plans that address the unique needs and goals of each patient.
- Foster strong relationships with patients, families, and interdisciplinary teams to ensure comprehensive care coordination.
- Utilize advanced clinical knowledge and analytical skills to identify opportunities for quality improvement and resource optimization.
- Collaborate with physicians, nurses, and other stakeholders to develop and implement evidence-based practices that enhance patient outcomes and reduce costs.
- Stay up-to-date with the latest clinical guidelines, research, and best practices to ensure delivery of cutting-edge care.
The Ideal Candidate:
- Current RN licensure with a minimum of 2 years of experience in med/surg, long-term acute care, or a related field.
- Demonstrated expertise in case management, care coordination, and disease management.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Aptitude for data analysis and problem-solving, with the ability to navigate electronic health records and other clinical systems.
