Mass General Brigham, a leading healthcare organization in the region, is seeking an experienced Registered Nurse (RN) to fill the role of Case Manager. This dynamic position plays a critical part in ensuring the seamless coordination of patient care across the continuum.

About the Role

The successful candidate will be responsible for facilitating effective communication and collaboration among healthcare professionals, patients, and families to achieve optimal health outcomes. As a Case Manager, you will work closely with patients, physicians, nurses, and other healthcare teams to assess individual needs, develop personalized care plans, and coordinate necessary interventions.

Key Responsibilities
  • Facilitate case management activities, including assessing patient needs, developing care plans, and coordinating services.
  • Collaborate with multidisciplinary teams to ensure comprehensive care delivery and address any barriers that may impact patient outcomes.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and coordinated care.
  • Monitor and evaluate patient progress, making adjustments to care plans as needed to ensure optimal health outcomes.
  • Promote evidence-based practices and adhere to established standards of care to ensure high-quality patient care.
Requirements

To be considered for this opportunity, applicants should possess:

  • A current RN license and relevant experience in medical/surgical, long-term acute care, or similar fields.
  • Strong analytical, data management, and PC skills.
  • Excellent interpersonal and communication skills, with the ability to negotiate and advocate on behalf of patients.
  • Knowledge of third-party payer systems and levels of care.
  • Familiarity with principles related to release of information and maintenance of confidentiality.
Preferred Qualifications

Applicants with prior home care, emergency department, or ICU experience are desirable, as are those holding BSN degrees or certifications in case management. Knowledge of Interqual, Milliman, and Midas systems is also beneficial.
