Healthcare Case Manager

Mass General Brigham seeks a skilled Healthcare Case Manager to join our team in Dover, New Hampshire. As a key member of our healthcare team, you will be responsible for coordinating patient care along the continuum, ensuring seamless transitions between healthcare settings.
We offer an estimated salary range of $65,000-$90,000 per year, depending on experience and qualifications. In this role, you will facilitate coordination of care for patients, meet with patients and families to assess needs and develop individualized plans, and collaborate with healthcare teams to ensure optimal patient outcomes.

Responsibilities- Coordinate patient care along the continuum, ensuring seamless transitions between healthcare settings.
- Facilitate coordination of care for patients, including referrals for home healthcare, hospice, DME, and pharmacy services.
- Meet with patients and families to assess needs and develop individualized plans in collaboration with healthcare providers.
- Collaborate with healthcare teams, including physicians, nurses, and other healthcare professionals, to ensure optimal patient outcomes.
- Document relevant discharge planning information in medical records according to department standards.
- Bachelor's degree in Nursing (BSN) or related field.
- Current and valid RN license.
- Minimum 2 years of experience in Med/Surg, Long Term Acute Care, or similar field.
- Prior experience in Homecare, Emergency Department, or ICU settings preferred.
- Strong analytical, data management, and PC skills.
- Knowledge of third-party payer systems and levels of care.
- Ability to prioritize multiple tasks and role components.
- Excellent interpersonal communication and negotiation skills.
