Job Overview

We are seeking a highly skilled Registered Nurse Case Manager to provide community-based care coordination and management services for our clients in the state of Delaware. This is a full-time, 3-month contract-to-hire role that offers a unique opportunity to make a meaningful difference in the lives of our members.

About the Role

As a Registered Nurse Case Manager, you will be responsible for traveling to members' homes, nursing facilities, and other community-based settings to conduct face-to-face needs assessments and subsequent telephonic contacts. You will work closely with healthcare professionals, social workers, and other stakeholders to develop and implement personalized care plans that address the complex needs of our members.

Your primary objective will be to ensure that our members receive high-quality, person-centered care that meets their physical, behavioral, and social needs. You will also be responsible for coordinating care across the continuum of services, including hospitalization, rehabilitation, and long-term care settings.

Key Responsibilities

* Conduct comprehensive needs assessments and develop individualized care plans that meet the unique needs of each member
* Coordinate care with healthcare providers, social workers, and other stakeholders to ensure seamless transitions and optimal outcomes
* Develop and maintain relationships with community resources and service providers to ensure access to necessary care and support
* Provide education and support to members and their caregivers on health care needs, benefits, and resources
* Collaborate with interdisciplinary teams to identify and address gaps in care and develop strategies to improve member outcomes
* Document all case management activities and interventions in accordance with regulatory requirements and company policies

Requirements

* Current Registered Nurse license in the state of Delaware
* 5+ years of experience in case management and discharge planning, including experience working with geriatric populations and individuals with complex needs
* Strong knowledge of community resources and services, including home health, hospice, and long-term care options
* Excellent communication and interpersonal skills, with ability to build strong relationships with members, families, and healthcare professionals
* Ability to work independently and as part of an interdisciplinary team
* Proficiency in electronic documentation systems and Microsoft Office Suite
