About the Role

We are seeking a skilled RN Case Manager to join our team at Mass General Brigham. This per diem / prn position is located in Dover, New Hampshire and offers a unique opportunity to work with patients and families in a dynamic healthcare environment.

Job Summary

The Discharge Planner RN is responsible for coordinating patient care transitions, collaborating with the interdisciplinary care team, and ensuring seamless communication between patients, families, physicians, and payers.

Key Responsibilities

  • Assess patients for discharge needs, develop individualized plans, and remove barriers to care transitions.
  • Facilitate coordination of care along the healthcare continuum, making referrals for post-acute care services as needed.
  • Demonstrate professionalism, leadership skills, and excellent time management, while maintaining close communication with directors and colleagues.
  • Collect data related to clinical pathway variances, adverse drug reactions, and infection control issues.

Requirements

  • 3 years of nursing experience, preferably in home health or case management.
  • RN degree, with Bachelors preferred; certification in case management is also desirable.
  • Strong clinical assessment skills, knowledge of medical standards of care, and ability to interrelate with physicians and hospital personnel.
