**Location:** Richmond, VA or Remote (with occasional travel)
**Job Type:** Full-time
**Salary:** $50,000 - $65,000 per year
We are seeking a dedicated and organized Care Coordinator to join our team at UMFS. As a Care Coordinator, you will play a critical role in ensuring that youth and families receive the necessary services and supports to achieve their goals and promote positive outcomes.
In this role, you will be responsible for coordinating care services, communicating with stakeholders, and maintaining accurate records. You will also work closely with a multidisciplinary team to ensure comprehensive care coordination and continuous improvement.
The ideal candidate will have a bachelor's degree in social work, psychology, or a related field, as well as 2+ years of experience working with children and families in a mental health or social services setting. We are looking for someone who is detail-oriented and able to prioritize multiple tasks and deadlines.
**Responsibilities:*
