Nurse Care Coordinator

Capitol Home Health is seeking a skilled Registered Nurse to join our team. As a Home Health Nurse Specialist, you will be responsible for planning, organizing, and directing home care services. You will work closely with patients and their families to provide high-quality care and support.
Key Responsibilities
- Plan and direct home care services to meet the needs of patients and their families
- Collaborate with healthcare teams to ensure seamless care transitions
- Provide education and support to patients and their families on health management and self-care
- Conduct regular assessments and evaluations to ensure patient safety and well-being
Requirements
- Current and unencumbered RN license in the state of employment
- Bachelor's degree in Nursing from an accredited program
- One to two years of recent acute care experience in an institutional setting
- One year of home health care experience preferred
- Valid CPR certification
- Excellent observation, verbal, and written communication skills
- Problem-solving skills and basic math skills
