Job Overview

Position: Registered Nurse - PRN

Shifts: Days or Nights/12 Hour Shifts/ Inpatient Center


Role Summary:

The primary role of the Registered Nurse is to deliver nursing care to patients of various age groups within the inpatient facility. This includes collaborating with the interdisciplinary team, patients, families, and referring agencies to ensure comprehensive care coordination.

Qualifications:

  • Education: Must be a graduate from an accredited Diploma, Associate, or Baccalaureate Nursing program.
  • Licensure: A valid Texas State Registered Nurse license and a current Texas Driver's License are required.
  • Experience: A minimum of two years in a clinical nursing environment is preferred, with hospice experience being advantageous.
  • Skills: Proficient nursing skills in accordance with established standards of practice, excellent interpersonal abilities, and documentation of Hepatitis consent/declination. Current CPR certification is mandatory. Specialized expertise in palliative and end-of-life care is preferred.
  • Transportation: Must possess reliable transportation and valid auto liability insurance.

Key Responsibilities:

  • Conduct thorough assessments of hospice patients to identify their physical, psychosocial, and environmental needs, supported by documentation and team evaluations.
  • Perform initial and ongoing comprehensive evaluations regarding the impact of terminal illnesses on patients' physical and emotional well-being.
  • Educate patients and families about disease processes, self-care methods, nutritional needs, and end-of-life care, while providing training to other staff as necessary.
  • Develop, implement, and document care plans to ensure quality and continuity of patient care.
  • Prepare clinical and progress notes that reflect advancements toward established care goals.
  • Apply infection control principles and standard precautions in all patient care activities to minimize risks.
  • Offer clinical guidance to Hospice Aides and LVNs to maintain high-quality service delivery.
  • Facilitate effective communication among patients, families, team members, and other healthcare professionals.
  • Ensure that quality patient care is consistently delivered with appropriate documentation.
  • Monitor assigned cases to confirm compliance with third-party payer requirements.
  • Demonstrate proficiency in the use of all equipment utilized in patient care.
  • Exhibit a commitment to professional development and competency in nursing practice.
  • Uphold the philosophy and administrative policies of Hospice of South Texas.
