Job Summary

We are seeking a highly organized and detail-oriented Program Service Coordinator to join our team at Sevita. As a key member of our program, you will be responsible for providing administrative support and program coordination, ensuring the smooth delivery of services to our participants.

Key Responsibilities
  • Program Coordination
    • Develop and implement individualized programs to meet the unique needs of our participants, optimizing their potential for community integration.
    • Assist participants in understanding program goals and processes, ensuring they receive the necessary support and resources.
    • Act as a liaison between families and our program, providing information and resources as requested.
  • Administrative Support
    • Organize and chair program participant conferences, ensuring effective communication and collaboration.
    • Review and ensure the accuracy of participant reports, documentation, and record-keeping.
    • Coordinate discharge planning, ensuring a smooth transition for participants.
  • Communication and Collaboration
    • Communicate effectively with team members, families, and community resources to ensure quality care and support.
    • Attend required meetings, including clinical team meetings and supervision sessions.
Requirements
  • Education
    • High School diploma or equivalent; Associate or Bachelor's Degree in a Social Service-related field preferred.
  • Experience
    • Two years of direct or indirect support services to persons with disabilities in a medical and/or rehabilitation setting.
  • Skills
    • Strong attention to detail, organizational skills, and ability to multi-task to meet deadlines.
    • Effective communication skills to manage relationships.
    • Self-motivated and collaborative; a team player.
    • Reliable, responsible attitude and a compassionate approach.
