Under the supervision of the Team Leader/Program Director, and in accordance with the guidelines established by the NYS OMH for the Mental Health Comprehensive Medicaid Case Management Program (MH CMCM), this role is integral to a multi-disciplinary treatment team. The primary focus is to assist individuals with serious and persistent mental health challenges in achieving and sustaining stable living, working, and social environments within the community. This is accomplished by facilitating access to essential medical, social, psychosocial, educational, financial, vocational, and other supportive services.

THIS IS A FIELD OPPORTUNITY

KEY RESPONSIBILITIES:

Deliver counseling, treatment, rehabilitation, case management, and support services in community settings (homes, shelters, streets) as part of a mobile multi-disciplinary team for individuals with mental health issues who have experienced decompensation and relapse due to inadequate follow-through with outpatient care. Take primary clinical management responsibility for a caseload of 10-15 adults with serious and persistent mental illness, who may also present with co-occurring substance use or other disorders, within a 68-person ACT Team. Lead the integration of family goals and services with the responsibilities of all team members, providing family psycho-education both individually and in group settings. Support consumers in achieving or maintaining housing, employment, functional social relationships, and alleviation of psychiatric symptoms through effective medication management. Engage assertively and provide mental health services in the community where the service population resides, with up to 80% of the weekly work schedule dedicated to fieldwork. Maintain continuous (24-hour) responsibility for individuals enrolled in the ACT Team caseload. Deliver all services in a culturally relevant and competent manner. Keep accurate and current Case Records, including but not limited to progress notes, service plans, assessments, and contact logs, in compliance with all applicable OMH, Medicaid, and ICL regulations and guidelines. Offer support and psychoeducation to family members, friends, and other collateral individuals. Facilitate referrals and engagement support for services not directly provided by the ACT Team, including medical services, participation in psychosocial clubhouses, self-help groups, vocational training, and placement. Provide timely and accurate reports to other providers and agencies, ensuring adherence to federal, state, and city regulatory and agency confidentiality guidelines. Participate in individual and group supervision, treatment team meetings, case conferences, in-service training, and other staff development activities as assigned. Adhere to all ICL, OMH, and other applicable rules, procedures, or laws regarding incident reporting, including the reporting of suspected child abuse and neglect. Perform other related duties as assigned by the Team Leader/Program Director.

Minimum Qualifications:

A Master's Degree in a discipline recognized by the New York State Office of Mental Health as qualifying for professional clinical staff, such as Social Work, Psychology, Occupational Therapy, Rehabilitation Counseling, Pastoral Counseling, Creative Arts Therapy, or Therapeutic Recreation. An applicable professional license issued by the NYS Education Department is strongly preferred. Additionally, at least one year of specialized experience or training in family assessment and intervention, psychoeducation, or other family support services, including cognitive-behavioral strategies, is required.
