Role Purpose:
The Lead Community Housing Advocate plays a vital role in engaging individuals experiencing homelessness through proactive outreach, employing a harm reduction strategy. This position acts as a case manager for adults facing street homelessness, aiming to facilitate their transition into both temporary and permanent housing solutions. Collaboration across teams and organizations is essential to achieve this objective.

Work Schedule:
Monday to Friday, 9:00 AM to 5:00 PM

Key Responsibilities:
  • Act as a case manager for individuals experiencing homelessness, focusing on securing transitional and permanent housing.
  • Conduct intake assessments, develop service plans, advocate for clients, and provide referral and escort services.
  • Maintain comprehensive client records and input data into relevant databases using both handheld and desktop devices.
  • Offer ongoing case management for clients who have been placed in housing to ensure retention and track outcomes.
  • Participate in team meetings, case discussions, training sessions, and supervision meetings; engage in program evaluation and enhancement.
  • Coordinate with evening and weekend staff, as well as personnel from other agencies, to ensure seamless client services.
  • Engage in outreach activities to identify and connect with homeless individuals.
  • Operate the outreach vehicle as needed.
  • Contribute to annual counts of homeless individuals living on the streets, including overnight assessments.
  • Perform additional duties as required.
Qualifications:
  • Bachelor's degree is preferred.
  • Experience working with homeless or mentally ill populations is advantageous.
  • A valid driver's license with a clean driving record that meets agency insurance standards.
  • Willingness to work flexible hours, including evenings, overnight shifts, and weekends as necessary.
Skills and Competencies:
  • Knowledge of housing-first and harm reduction models.
  • Ability to manage multiple tasks effectively.
  • Excellent interpersonal communication skills.
  • Strong written and verbal communication abilities.
  • Team-oriented mindset.
Technical Proficiency:
  • Proficient in Microsoft Outlook.
  • Able to utilize or learn to navigate databases such as Streetsmart, MOcapp, and CARES.
Physical Demands:
  • Frequent walking and standing required.
  • Ability to lift items weighing up to 30 lbs.
Work Environment:
  • Work will be conducted both indoors and outdoors, in various weather conditions.
This position is part of a unionized workforce under the Collective Bargaining Agreement between Goddard Riverside and the United Service Workers Union Local 74. The agreement outlines the terms of employment, including wage increases, paid time off, and other benefits. All employees in union positions are subject to this agreement, which includes the collection of union dues.
