Job Summary

We are seeking a highly skilled and compassionate Senior Case Manager - CCBHC to join our team at Outreach Development Corp. As a key member of our staff, you will play a vital role in providing case management and crisis intervention services to individuals in need.

Key Responsibilities
  • Provide case management services to clients, including coordination of treatment services and support groups.
  • Facilitate life skills support groups and advocate on behalf of clients to obtain needed services.
  • Interact with various agencies and systems to ensure clients receive necessary support and services.
  • Conduct ongoing case management related to housing, transportation, childcare, food, and other essential needs.
  • Maintain communication with direct service providers and vocational service providers.
  • Submit weekly schedules and monitor effective time utilization.
  • Participate in staff and community meetings as needed.
  • Provide crisis intervention and group work services, as well as backup coverage when required.
  • Maintain computer skills necessary for optimal job performance.
  • Ensure delivery of quality care in accordance with agency Medicaid Compliance Plan and systems of accountability.

  • Ability to maintain confidentiality and handle sensitive information.
Compensation

The salary for this position is $38,000 - $50,000 per year, depending on experience.
