Role Overview:
The Patient Care Coordinator plays a crucial role in the healthcare team, ensuring seamless patient experiences by managing appointments, addressing billing inquiries, and overseeing medical records.

Key Responsibilities:
1. Facilitate daily requests for referrals and authorizations.
2. Maintain meticulous tracking of all referral processes.
3. Collaborate with healthcare providers to ensure comprehensive documentation.
4. Assist in coordinating patient care with physicians as needed.
5. Communicate with health plans to confirm prior approval requirements.
6. Compile relevant clinical information for specialists based on referral guidelines.
7. Ensure timely resolution of referrals and authorizations.
8. Serve as the primary contact for patient inquiries regarding referrals.
9. Work closely with Centralized Services to address patient needs.
10. Build and maintain relationships with service providers for effective care coordination.

Qualifications:
Required:
- High School Diploma or equivalent.
- At least 1 year of experience with Microsoft Office.

Preferred:
- Experience in a medical front desk role.
- Customer service experience of 1 year or more.
- Familiarity with electronic medical records.
- Knowledge of health plan insurance processes.
- Proficiency in data analysis software.
- Bilingual candidates (English/Spanish) are encouraged to apply.
