Job Overview

As a vital member of the population health team, the Care Coordinator plays a crucial role in managing incoming referrals and monitoring outreach efforts. This position involves making proactive outreach calls and utilizing digital communication tools to schedule patient appointments, share resources, and facilitate care coordination. Additionally, the Care Coordinator provides essential instructions for class and visit preparations to enhance patient experiences.

In this role, you will handle incoming inquiries from patients, healthcare providers, and clinic personnel, ensuring effective communication and support. You will also compile and analyze data to prepare and submit reports that reflect program performance and organizational metrics. Responsibilities include scheduling educational sessions and managing venue logistics, as well as providing financial counseling and addressing routine billing inquiries.

Key Responsibilities:
  • Coordinate and manage patient referrals and outreach activities.
  • Make outgoing calls and send messages to facilitate appointment scheduling and resource sharing.
  • Answer incoming calls and provide assistance to patients and healthcare staff.
  • Compile data and generate reports for performance evaluation.
  • Organize educational classes and manage logistical requirements.
  • Assist with financial counseling and resolve billing issues.
  • Perform clerical duties as assigned by the supervisor.
Qualifications:

Required:
  • Minimum of 2 years of experience in a healthcare setting, particularly in ambulatory care.
  • 2 years of driving experience, with compliance to state driving regulations.
Preferred:
  • Associate's Degree in a relevant field.
  • Experience with Excel and data management.
  • Familiarity with coding systems such as ICD-9, ICD-10, CPT, and HCPCS.
  • Prior experience with electronic medical records (EMR).
  • Experience in a liaison role among patients, providers, and staff.
