Join Our Team as a Client Care Coordinator
Community Care, Inc. is seeking a dedicated Client Care Coordinator. This fulfilling role is ideal for individuals who thrive in collaborative environments, focusing on community-based case management while contributing to a mission-driven organization.
At Community Care, our goal is to assist elderly individuals and adults with physical and intellectual disabilities in maintaining their independence within the community.
This full-time position features a weekday schedule and includes a generous sign-on bonus.
Key Responsibilities of the Client Care Coordinator:
- Oversee a designated caseload of program participants in partnership with a Registered Nurse Care Manager.
- Conduct thorough assessments of members, develop individualized care plans, and perform functional evaluations.
- Ensure continuous care coordination and monitor the quality of services provided.
- Maintain accurate documentation in compliance with established standards.
Qualifications for the Client Care Coordinator:
Education: A Bachelor's Degree in Human Services or a closely related field is required. Candidates with a minimum of three years of experience working with seniors or adults with disabilities may qualify without a human services degree.
Experience: Preferred candidates will have at least one year of experience working with older adults or individuals with physical and/or intellectual disabilities in a social service setting.
Certification: Eligibility for long-term care functional screener certification is necessary.
Travel: This role requires regular local travel, necessitating the use of a personal vehicle, a valid driver's license, and proof of insurance coverage.
Work Schedule:
- Monday to Friday, 8:00 AM - 4:30 PM
