ProHealth Care RN Complex Care Coordinator Job

We are seeking a skilled RN Complex Care Coordinator to join our team at ProHealth Care. As a RN Complex Care Coordinator, you will play a critical role in ensuring the smooth transition of patients from inpatient discharge to the community setting.

Key Responsibilities:

  • Build rapport with patients over the phone to identify and address concerns that may be a barrier to a smooth transition.
  • Collaborate with the department and across the care continuum to prioritize and adapt to fluctuating patient volume/needs.
  • Work closely with patients, families, and healthcare teams to ensure seamless care transitions.
