About the Role

Lutheran Social Services of Wisconsin and Upper Michigan is seeking a dedicated Care Coordinator to join our team in the Birth to Three Program. As a Care Coordinator, you will play a vital role in supporting families of children with developmental delays or disabilities. Your primary responsibility will be to enhance the child's development and empower the family's knowledge, skills, and abilities as they interact with and raise their child.

Key Responsibilities
  • Assess client needs utilizing the required assessment tool(s) and re-assess as needed, at least annually.
  • Develop and implement comprehensive and individualized service plans in collaboration with clients, their families, and other service providers.
  • Coordinate and authorize services according to the service plan, ensuring alignment with the client's needs and goals.
  • Provide services to assigned clients in accordance with the service plan, promoting client independence and growth.
  • Collaborate with clients, service teams, and clinical supervisors to determine interventions and ensure effective communication.
  • Document services provided in a neat, legible, accurate, objective, and consistent manner, adhering to established timeframes and expectations.
  • Establish and maintain effective working relationships with clients, service teams, and clinical supervisors.
  • Participate in staff development programs, including in-service training, staff meetings, and professional seminars, to enhance knowledge and skills.
Requirements
  • Bachelor's degree in social work, child development, or a related field.
  • One year of supervised experience working with families with special needs preferred.
  • Valid driver's license and reliable transportation to perform essential duties.
  • Ability to work in a community environment, including home, office, and community settings.
  • Ability to react appropriately in times of crisis, including verbal aggression.
