We are currently seeking a skilled Licensed Practical Nurse to join our team as a Healthcare Liaison LPN. In this role, you will work closely with our interdisciplinary team to coordinate care and resources for patients participating in the CEASE program. Your primary responsibilities will include triaging patients who present with interest in smoking cessation information, evaluating referred patients for readiness to quit smoking, and establishing daily, weekly, and monthly goals with enrolled CEASE Program patients. You will also conduct health/educational assessments, re-evaluate daily, weekly, and monthly goals, and facilitate in-person individual/group sessions as scheduled per team. Additionally, you will participate in case conferences with the interdisciplinary team and provide supporting facts where necessary. As a Healthcare Liaison LPN, you will also assist physicians with individual health education, prepare monthly, quarterly, and annual reports, and develop policies, procedures, and clinical protocols. We offer a competitive salary of approximately $65,000 per year, based on industry standards for a nurse in this role and location.
