About the RoleThis is an excellent opportunity for a skilled Clinical Liaison to join our growing team of professionals at Hospice of Virginia. The successful candidate will be responsible for providing high-quality care coordination and advocacy for our patients and their families.
Key Responsibilities- Care Coordination: Work closely with patients, families, and healthcare providers to ensure seamless transitions and optimal care
- Service Development: Collaborate with interdisciplinary teams to develop and implement new service lines and initiatives
- Education and Outreach: Present educational in-service programs to referral sources, community partners, and other agency departments
- Regulatory Compliance: Stay up-to-date on agency services, payors, regulations, and industry trends to ensure compliance and quality care
- Active RN licensure in the state of practice
- Current CPR, First Aid Certification
- 1-2 years of experience in hospice care or a related field
- Valid driver's license and proof of insurance required
