Our organization is seeking a Licensed Vocational Nurse (LVN) to contribute to our mission.

Key Responsibilities

In alignment with established standards and policies:

  • Contributes to the formulation and upkeep of care plans.
    • Assists in the creation, execution, and communication of patient care strategies.
    • Regularly updates the Staff Registered Nurse and/or Clinical Services Coordinator on patient needs.
    • Alerts the Staff Registered Nurse and/or Clinical Services Coordinator immediately of any significant changes in patient status.
    • Engages in case discussions as per organizational guidelines.
    • Involves patients and their families in the care planning process whenever feasible.
    • Educates patients and families about their rights and care plans.
    • Addresses patient inquiries and concerns promptly and professionally.
  • Executes current nursing practices.
    • Evaluates patient conditions during each home health visit.
    • Implements nursing interventions tailored to patient needs and changing conditions.
    • Transcribes and evaluates physician orders accurately.
    • Keeps precise and timely clinical documentation.
    • Adheres to infection control and safety protocols, reporting any hazards promptly.
    • Maintains confidentiality of patient information.
    • Ensures compliance with health and safety regulations.
  • Utilizes equipment and supplies effectively and safely.
    • Communicates patient equipment and supply needs to relevant personnel.
    • Demonstrates efficient use of medical equipment and supplies.
    • Reports any malfunctioning equipment or supply shortages.
    • Informs patients and families about the safe use of equipment and supplies.
    • Provides guidance on reimbursement processes for necessary equipment.
  • Engages in professional development.
    • Participates in ongoing educational initiatives.
    • Maintains up-to-date knowledge of nursing principles.
    • Contributes to the development and revision of clinical policies.
    • Assists in creating and delivering educational programs as needed.
    • Maintains a valid professional license and good standing.
    • Attends in-service training sessions.
    • Completes additional responsibilities as assigned.
