We are seeking a Licensed Vocational Nurse (LVN) to contribute to our community.

Key Responsibilities

In alignment with established agency standards and clinical policies:

  • Engage in the creation and upkeep of treatment plans.
    • Assist in the formulation, execution, and communication of patient care plans.
    • Relay identified patient needs to the Staff Registered Nurse and/or Clinical Services Coordinator promptly.
    • Alert the Staff Registered Nurse and/or Clinical Services Coordinator immediately regarding any significant changes in patient conditions.
    • Participate in case discussions as per agency guidelines.
    • Involve patients and their families in the development and execution of care plans whenever feasible.
    • Provide information about patient rights, responsibilities, and treatment plans to patients and their families.
    • Address and communicate patient inquiries, concerns, and complaints in a timely and professional manner.
  • Execute current nursing practices.
    • Assess patient conditions during each home health care visit.
    • Implement skilled nursing interventions to address patient needs and evolving conditions.
    • Transcribe, execute, and evaluate physician orders.
    • Maintain accurate, complete, and timely clinical documentation.
    • Implement appropriate infection control and safety protocols, reporting all hazards and incidents promptly.
    • Ensure confidentiality of patient information.
    • Foster an agency environment compliant with federal, state, and local health and safety regulations.
  • Utilize equipment and supplies safely and effectively.
    • Communicate patient equipment and supply needs to relevant individuals consistently and promptly.
    • Demonstrate efficient and accurate use of equipment and supplies.
    • Report any malfunctioning equipment or insufficient supplies to the appropriate personnel.
    • Educate patients and families on the safe and effective use of equipment and supplies.
    • Provide information regarding reimbursement for necessary equipment and supplies to patients and families.
  • Engage in professional development.
    • Participate in ongoing educational programs.
    • Demonstrate current knowledge of nursing principles.
    • Contribute to the development and revision of agency clinical and administrative policies.
    • Participate in the creation and presentation of educational programs as appropriate.
    • Maintain an active professional license and ensure good standing.
    • Attend agency in-service training sessions.
    • Fulfill additional responsibilities as assigned.
